Please notify all immunisations to the National Immunisation Registry (NIR), Health Promotion Board. NIR can be accessed via www.nir.hpb.gov.sg/ens/enslogin. Notification forms 550A can be requested from NIR via email hpb_nir@hpb.gov.sg.
